The Team Lead/Supervisor is directly responsible to the Owner/Administrator. She/he is responsible for the overall client service operations and assigned personnel supervision. You would not be alone, you would be supported by management and the agency to help you fulfill the duties that you are expected to complete. We would love to have a supervisor who has a background in caregiving and knows the challenges of being a caregiver. Also challenges for people who are older or have disabilities. We as an agency are looking to grow and are interviewing applicants immediately to fill this need.

Qualifications
  • English (Required)
  • Driver's License (Required)
  • Caregiver or DSP experience (Required)
  • Bathing assistance: 1 year (Preferred)
  • Must understand and respect client ethics and confidentiality of care for all clients
  • Experience in the health care field (Preferred)
  • Experience as a supervisor for people who have physical and mental disabilities (Preferred)
  • Must have on hire and maintain CPR and First Aid certification (Required) 
  • Must be computer literate (Required) 
  • Must possess effective communication skills and the ability to lead a team of personnel with a variety of backgrounds and educational experiences (Required) 
  • Must possess  good time management, problem-solving, and work ethic skills (Required) 
  • Must present a professional appearance and promote a positive work environment (Required)

This position advocates for the associate experience. This position also works on the process of setting appointments, meeting with new and existing employees, and overseeing the overall onboarding experience. This position assists new associates with the employment process and assesses candidate strengths and skills to facilitate a great match for the assignment and team.

RESPONSIBILITIES: 

1. Demonstrates support for the agency mission statement to promote quality, efficient, comprehensive, and effective client services.
2. Available to cover missed shifts when needed.  
3. Responsible to train/job shadow new applicants/employees on current jobs 
4. First contact for team members & clients when issues arise 
5. Does initial assessment and development of the client plan of care.  
6. Assures continuity of assigned caregivers to clients  
7. Ensures documentation is complete and accurately structured according to company guidelines.  
8. Assist Senior Management as needed within the program. 
9. Be able to talk to potential clients about Golden Heart to become future clients
10. Supervises and serves as a role model to all assigned personnel. 
11. Available on premise or via telecommunications to supervise and serves as a role model to all assigned personnel. 
12. Participates in and supports the agency’s performance improvement activities. 
13. Participates in employee orientation, competency testing, and in-services. 
14. Uses effective communication skills to follow-up on complaints by clients or personnel. 
15. Maintains confidentiality of client, employee, and agency operations.
16. Keeps all records up to date, organized, and ready for inspection at any time. 
17. Promotes compliance with all agency standards, policies, and procedures. 
18. Promotes client, personnel, and office safety precautions. 
19. Presents a professional appearance and promotes a positive work environment. 
20. Works w/ Scheduler/Client Care Coordinator client service assignments. 
21. Participates in the quarterly record review and works with personnel to initiate needed changes to maintain records in accordance with state and accreditation standards.
22. Reviews all admissions, transfers, discharges, and services to ensure that they meet agency policies and procedures and standards of practice. 
23. Facilitates referrals to the agency. 
24. Supervises Home Care Aids, Direct Support Professionals, and Homemakers in a frequency as required including peer review as applicable. All aspects of training (if provided), competency testing, direct observation, documentation review, and monitoring of required annual in-service training are included in the supervisory duties. 
25. Represents the agency at community functions. 
26. Participates in the development of the annual program evaluation and strategic plan. 
27. Participates in interview and selection of new personnel as delegated. 
28. Evaluates assigned personnel performance and provides corrective action if needed
29. Performs other duties as requested. 

As the Team Lead you will report to Management and/or to the Leadership team to report on all of the above including but not limited to: client concerns, staff concerns, activities, goals, updates, problems, and any special projects given.    

This employee must have strong organizational skills, be proficient in spreadsheets and word processing, and be computer literate, as well as having the ability to communicate and problem solve effectively. And finally, assist management as needed. 
Compensation: $17.50 - $19.00 per hour
