Job Title: Registered Nurse – Client Case Manager (In-Home Care Assessments)

Location: Prince William County, VA (including Manassas, Woodbridge, and surrounding areas)
Job Type: Per Diem (with future opportunity for Part-Time or Full-Time)
Compensation: Paid per completed assessment (Flat-rate per visit, with the possibility for rate adjustments if you become a part-time or full-time employee.)

About the Role:
We are currently seeking a Registered Nurse (RN) – Client Case Manager to perform in-home care assessments and support the development of individualized care plans for our clients. This position is best suited for someone adaptable, responsive, and passionate about ensuring safety and quality care in the home setting.

This is an on-call, flexible, per-visit role to start. You’ll be contacted when we have a client interested in services and will be expected to complete the in-home assessment and care plan in a timely manner. As the agency expands, there will be opportunities to grow with us into a part-time or full-time RN role.

Key Responsibilities:

  • Conduct in-home client assessments to evaluate the safety, needs, and support required in the home environment.
  • Complete care assessments and client care plans via DocuSign, outlining client needs, preferences, and safety concerns.
  • Make professional care recommendations based on assessment findings.
  • Collaborate with the agency owner and office team to ensure care plans are properly implemented and supported.
  • Provide caregiver training as needed and offer clinical guidance on client concerns.
  • Occasionally supervise HHAs and CNAs and perform follow-up evaluations if required.
  • Be available by phone or email for input on client care issues as they arise.
What We're Looking For:
  • Active State of Virginia or Compact RN license (required)
  • 3–5 years of relevant clinical nursing experience
  • Strong interpersonal and communication skills
  • Reliable transportation and willingness to travel throughout Prince William County (from Manassas to Woodbridge, and surrounding areas)
  • Organized, detail-oriented, and adaptable to client needs on short notice
  • Passion for in-home care and commitment to long-term client wellness
