Shift is Monday-Friday, 4pm-7pm in Bountiful for 3 Special Need's patients.
Responsibilities
- Perform client personal care needs and support other clients and family every-day-needs as supported by the hospice plan of care
- Record and report important health information including temperature, heart rate, respiration rate and/or oxygen saturation, and blood pressure as indicated in the plan of care
- Communicate effectively and respectfully with a diverse client population and co-workers
- Work under the supervision of a nurse:
- Promptly communicating any family or client concerns or changes in status
- Consulting with a nurse with regard to suggested changes in treatment
- Submit required documentation in a timely manner
- On call every other weekend
- Current State CNA Certification in good standing
- Current CPR with BLS card
- Valid driver’s license, at least state minimum auto insurance, and an operational vehicle
- Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing hospice operations
- Great organization skills and an ability to work without direct supervision
- Ability to remain calm in emergency or crisis situations, and to be close to clients who may be actively dying
- Commitment to adhere to professional boundaries
- Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system
Physical Requirements
- Duties require extensive standing, walking, and sitting
- Requires lifting, positioning, pushing, and/or assistance with transferring patients
- Requires frequent reaching, stooping, bending, kneeling, and crouching
