Responsibilities:

Client Assessments: Conduct thorough evaluations of client needs.
Care Plans: Develop comprehensive care plans tailored to each client.
Evaluations: Perform 30-day check-in evaluations and 60-day re-assessments.
Supervisory Visits: Oversee and ensure the quality of care provided.
Education: Provide essential education to clients and caregivers.
Standards of Care: Ensure the delivery of proper care adhering to RN standards.
Communication: Maintain regular communication with patients, families, caregivers, pharmacies, doctors' offices, and medical services.

Qualifications:

License: Current New Jersey Registered Nurse (RN) license, preferably with a BSN.
Experience: Proven experience in patient care, with a preference for Home Care.
Transportation: Valid driver's license and reliable mode of transportation.
Travel: Able to travel to clients' homes throughout Hudson and parts of Bergen counties.
Skills: Knowledge of medical terminology and procedures.
Communication: Excellent written and verbal communication skills.
Interpersonal Skills: Strong interpersonal skills and a compassionate approach to patient care.
Independence: Self-motivated, organized, and able to work independently.
Vaccination: Fully vaccinated and boosted for COVID-19.

Compensation: $48.00 - $51.00 per hour
