Job Summary
We are seeking a part-time RN Field Supervisor to work in a fast paced environment, where time management is essential. The Case Manager position will ensure the health and well being of our clients are met by ensuring quality care. In alignment with company mission, vision and values, the RN Case Manager coordinates, implements, and manages care plans and Case Management services to out line a direction for our Caregivers to follow.

As a Registered Nurse (RN) you will utilize your great attention to detail and caring heart to develop interpersonal relationships between yourself and clients. This position will require someone who is able to observe and communicate efficiently and effectively in your patient's best interest. 

To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems. 

Responsibilities: 
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 
  • Develop and implement a written Care Plan for each client, with written approval by the client/ designated family member. 
  • Able to interact effectively and professionally with client and/or support network
  • Assure that the Client Service Agreement packet is completed for each new client, with the original copy kept for the client file and a copy given to the client/designated family member; collect fees for initial services at the time of assessment, or by the first day of service.
  • Assure the Care Plan is reviewed on-site, updated, approved and signed by the client/ designated family member and the appropriate agency personnel.
  • Provide each Caregiver with the appropriate training needed to succeed 
  • Assure Caregivers are complying with the Care Plan 
  • Document any changes in patient conditions and report on changes 
  • Maintain accurate records of patient care and discuss observations with supervisor 
  • Be in an On-Call Rotation for emergent needs
Qualifications
  • Currently licensed Registered Nurse (RN) in Minnesota
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver’s license 
  • Team player with a compassionate spirit
Compensation: $30.00 - $35.00 per hour
