Case Management Liaison $19-$21 per hour

$19 to $21 for a Support Services Liaison. Prefer applicants who have worked as a DSP in the past.
Preference given to those who have worked as a House Manager for TWO years.
Position Summary:
Provides case management and coordination of care for assigned support services clients in a 5 county area to ensure quality care delivery within the Agency’s scope of services, policies and procedures, State and Federal laws and Regulations.
Minimum Qualifications:
· High school graduate
· Has six (6) months experience with support services agency.
· Ability to supervise and direct care team members.
· Excellent verbal and written communication skills and strong interpersonal skills.
· Computer literate.
· Organized
· Self-motivated
Essential Position Functions:
· With client and or client’s Representative, develops and evaluates the Client’s Service Plan.
· Reviews and revises the Service plan.
· Attends and actively participates in case management meetings, as needed, in person, by phone, or by other Agency approved method of communication.
· Provides services in accordance with Agency scope of services, policies and procedures, State, Federal, laws and Regulations.
· Demonstrates effective organizational and time management skills.
· Demonstrates strong, effective, written, and verbal communication skills, including good interpersonal skills, that facilitate tactful and effective engagement with staff, clients, family members, the community, and other Agency stakeholders.
· Consistently demonstrates competency with personal care attendant tasks.
· Maintains confidentiality.
· Maintains infection prevention and control practices.
