Job Title: Licensed Practical Nurse (LPN), Healthcare Services Representative

Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with daily activities.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Educate patients and their families on health conditions, treatment plans, and medication usage.
- Maintain accurate and up-to-date patient records, documenting care provided and any changes in patient condition.
- Assist in the coordination of patient appointments, referrals, and follow-up care with other healthcare providers.
- Ensure compliance with healthcare regulations, safety standards, and infection control protocols.
Minimum Qualifications:
- Current and valid LPN license in the state of practice.
- Minimum of 1-2 years of clinical experience in a healthcare setting, preferably in a hospital or outpatient clinic.
- Strong communication skills with the ability to effectively interact with patients, families, and healthcare team members.
- Proficiency in electronic medical record (EMR) systems and basic computer skills.
- Ability to work independently and as part of a team in a fast-paced environment.
- Strong organizational and time management skills, with attention to detail.
Compensation: $20.00 - $25.00 per hour
