Position: Clinical Pharmacist – Bone Marrow Transplant (BMT)
Location: Gainesville, FL

Job Description:
We are seeking an experienced Clinical Pharmacist to join our Bone Marrow Transplant (BMT) team in Gainesville, FL. The Clinical Pharmacist will play a crucial role in managing and optimizing medication therapy for patients undergoing bone marrow transplants and related treatments. This role involves collaborating with the multidisciplinary care team, providing expert pharmacological guidance, and ensuring the safe and effective use of medications.

Key Responsibilities:

  • Review and interpret medication orders for bone marrow transplant patients, including chemotherapy, immunosuppressants, and supportive care medications.
  • Develop and manage individualized medication therapy plans based on patient-specific factors, including drug interactions, contraindications, and pharmacokinetics.
  • Monitor patient responses to medications, adjust dosages, and provide recommendations for therapeutic changes as needed.
  • Provide drug information and education to patients, families, and healthcare providers regarding medication use, side effects, and potential interactions.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal medication management and patient outcomes.
  • Participate in the development and implementation of protocols and guidelines related to medication use in BMT patients.
  • Conduct medication utilization reviews and contribute to quality improvement initiatives related to drug therapy.
  • Maintain accurate and detailed records of medication therapy management and interventions in the electronic health record (EHR).
Qualifications:

  • Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Current Pharmacist license in the state of Florida.
  • Board Certification in Oncology Pharmacy (BCOP) preferred or willingness to obtain.
  • Minimum of 2 years of experience in clinical pharmacy, with a focus on oncology or bone marrow transplant preferred.
  • Strong knowledge of pharmacotherapy, drug interactions, and supportive care for BMT patients.
  • Excellent communication, analytical, and problem-solving skills.
  • Ability to work effectively in a collaborative, multidisciplinary team environment.
  • Proficiency with electronic health records (EHR) and clinical documentation.

Compensation: $61.00 - $71.00 per hour
