Job Summary: Our hospice agency is seeking a compassionate and qualified Medical Social Worker to join our team. The Medical Social Worker will provide counseling and support to hospice patients and their family members during and after medical treatment. The ideal candidate will be able to identify and address the emotional and social factors that impact a hospice patient's health and wellbeing at the end of life.
Responsibilities:
- Collaborate with the hospice interdisciplinary team, which includes physicians, nurses, and other healthcare professionals, to develop hospice care plans
- Conduct assessments of hospice patients' social and emotional needs and develop appropriate interventions
- Provide individual and group counseling to hospice patients and families on issues related to hospice care, chronic illness, and end-of-life care
- Connect hospice patients and their families with community resources and services
- Case-manage hospice patients who require ongoing support and assistance
- Document all interactions in the hospice patient's medical record according to departmental standards
- Participate in hospice team meetings and quality improvement initiatives as needed
Qualifications:
- Master's degree in Social Work from an accredited institution
- 2+ years of experience working in hospice, home health, or with chronically ill patients preferred
- State licensure as a Clinical Social Worker or eligibility to obtain licensure required
- Knowledge of hospice care, medical terminology, and medical social work theory and practice
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of a hospice interdisciplinary team
- Demonstrated commitment to diversity, equity, and inclusion in the workplace
