ComForCare Home Care - Greater Charlotte is a premier agency providing in-home care providers throughout Mecklenburg County. As a Registered Nurse at ComForCare, you will conduct thorough client assessments, create individualized care plans, implement interventions to achieve client goals, update care plans to track progress, monitor medication and health changes on a regular basis, maintain detailed client records, and coordinate referrals to other healthcare professionals as required.

What we are looking for:
- Graduate of an accredited school of professional nursing
- Current license as a Registered Nurse (RN) in North and South Carolina
- CPR certification
- Demonstrated knowledge of safety and infection control practices
- Mon-Fri/combination of in office and field work/10-20 hours a week
- Occasional weekend availability
- Must be proficient in the use of technology
What you will be doing:
- Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters
- Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings
- Providing interventions with a focus on achieving realistic client outcomes
- Reporting changes to update the plan of care to reflect progress towards goals and outcomes
- Reviewing medications as needed
- Maintaining client records showing systematic assessments, planning interventions, and evaluations
- Initiating referrals as needed to other health team members
Salary Range:
- $32 - $35 an hour
- Bi-weekly pay
