Health Centers Detroit Foundation, Inc. (HCD), is a non-profit, 501(c)3, three-site community health center organization founded in March of 2004 and was designated a Look-Alike in the Federally Qualified Health Center (FQHC) Look-Alike program in October of 2005. Our organization feels that though the 14 years of operations as a Look-Alike we have built an extraordinarily high quality primary care organization. Most of our physicians graduated from the Wayne State University primary care residency programs in Family Medicine, Internal Medicine, Pediatrics, and “Dual Certified” Internal Medicine/Pediatrics known as “Med/Peds” where they were recruited by HCD. Most have also served as chief residents within their respective residency programs. They bring expertise and conviction to delivering socially competent and effective care to our patients.

HCD operates three health centers. Two are located with in the City of Detroit. The 1st at 4201 St. Antoine UHC Suite 7A on the Detroit Medical Center campus, the 2nd at 7633 East Jefferson Avenue Suite 300. The 3rd site is just North of 8 Mile Road in the city of Southfield at 23077 Greenfield Suite 400. All three HCD health center sites are located in areas that are designated Medically Underserved Areas (MUA’s) by the Federal Government with scores in the 30’s and 40’s, very much below the 62 point base line to be considered a MUA. The MUA designation for the Detroit area has been in place since 1997.

HCD offers a full spectrum of primary care services to its patients, both directly and through strategic partnerships. It has a current staff comprised of eight physicians, a PhD Pharmacist specializing in diabetes counseling and a mental health counseling professional. HCD serves between eight to ten thousand unique patients a year.

HCD operates in close collaboration with the hospitals that comprise the Detroit Medical Center DMC/Tenet health system and the faculties that comprise the Wayne State University School of Medicine. The DMC/WSU SoM collaboration still remains the leading safety net health care provider systems in Southeast Michigan for the underserved and uninsured, as well as WSU SoM being the largest single campus medical school in the nation. HCD also has associations with McAuley Health Center | University of Detroit Mercy, University of Detroit Mercy Dental College, Detroit Area Council on Ageing, Detroit Wayne Mental Health Authority, Institute of Population Health, University Physicians Group, and others.

HCD's CEO is a founding member of, and still serves on, the South Western FQHC Council which is a collaborative group of FQHC's mostly located in the Detroit Metro and surrounding areas where they provide quality safety net medical services to the community. HCD is a member of the Michigan Primary Care Association which represents the 43 FQHC's in the State of Michigan.
