Schedule:
- Office hours: Monday – Friday, 9:00 AM – 6:00 PM
- Part-time hours to start, with potential for full-time employment based on performance and program needs.
Key Responsibilities:
- Provide case management, counseling, and support services to individuals and families within the community.
- Conduct assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to address client needs and goals.
- Facilitate group sessions, workshops, or community events as required.
- Maintain accurate and timely documentation in compliance with agency and regulatory standards.
Qualifications:
- Active LGSW license in Washington, DC.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Experience in community-based or outpatient settings is preferred.
- Commitment to serving diverse populations and promoting equity and inclusion.
