- Performs high quality, comprehensive, pre-visit planning to identify gaps in care, appropriate preventative screenings and timely completion of lab orders.
- Adheres to agency goals, objectives, standards of performance and policies and procedures.
- Acts as a resource within the clinic to staff members and patients in regards to patient care and patient education.
- Identifies patients that are due for follow up, assesses for any potential barriers to care, to include social determinants and encourages appropriate healthcare maintenance.
- Utilizes electronic health records to track labs, diagnostics, transitions of care and reviews patient outcomes utilizing referral logs.
- Reviews quality reports through various health insurance portals as assigned.
- Coordinates the integration of care management functions into patient care with other agencies, external service organizations and healthcare facilities.
- Provides leadership to the Patient Care Clinical Team, conducts regular team meetings and delivers team support as per the patient centered medical home model.
- Performs miscellaneous duties as assigned.
- Must be a graduate of an accredited school of nursing and licensed in the State of Texas or be a Certified Medical Assistant with previous medical office experience.
- Must have a valid driver’s license and a safe driving record and be able to pass a pre-employment physical and a criminal history background check.
- Bilingual (English/Spanish) ability is preferred.
- Ability to effectively communicate with patient population, providers, and staff while demonstrating a high degree of diplomacy and tact.
- Beginner to intermediate computing and phone skills.
- Willingness to demonstrate flexibility in regards to job duties and assignments.
- Ability to multi-task and work effectively in a fast paced environment.
- Cultural sensitivity to effectively work with ethnically diverse populations.
- Thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.
