Job Summary:
As a Care Coordinator, you will collaborate with patients, physicians, and healthcare teams to develop personalized care plans and ensure that each client receives the best care possible! You will play a key role in coordinating care and ensuring that goals are met effectively. If you're compassionate, reliable, and knowledgeable in healthcare practices, we want to hear from you!

Responsibilities:

  • Work closely with physicians, patients, families, and healthcare staff to create and monitor care plans
  • Review and update care plans as needed to ensure clients' needs are met
  • Develop and manage individualized care plans
  • Accompany clients to doctor and hospital visits
  • Set client goals and track progress toward achieving them
  • Recruit, train, and manage staff
  • Create schedules and manage a caseload of 30-40 clients
Qualifications:

  • Previous experience as a Care Coordinator or similar position(s) is preferred
  • Strong problem-solving and organizational skills
  • Ability to manage multiple tasks efficiently and prioritize effectively
  • Able to thrive in a fast-paced environment and make sound decisions quickly
  • Excellent interpersonal skills with a high level of compassion
  • Strong verbal and written communication skills
  • Medical assistant certification is a plus

Compensation: $23.00 - $24.00 per hour
