About the Role & Company:
Reports To: Administrator
Provide Supervision to: All direct care staff
- Contribute to the development of internal processes and strategies to ensure quality care delivery.
- Conduct in-home client visits such as start-of-care assessments, supervisory visits, and reassessment visits.
- Assist with staff hiring, orientation, training, and conducting annual performance evaluations.
- Receive referrals, assess service needs, and assign appropriate clinicians to cases.
- Attend clinical meetings to facilitate care coordination and meet regularly with clinical staff to provide guidance, set goals, and evaluate progress.
- Support staff, clients, and families by providing direction on healthcare resources and educating them on clinical policies.
- Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
- Help foster a culture of innovation, collaboration, and excellence within the team.
About You:
- Graduate of an accredited school of professional nursing. BSN preferred.
- Current license as a Registered Nurse in the state of MN with at least two (2) years of homecare experience and one (1) year of supervisory experience.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Must be a licensed driver with an automobile insured in accordance with Agency and state requirements.
- Current CPR certification.
