Your Role
We’re looking for an experienced and compassionate Home Health Physical Therapist to join our growing team. The right candidate will have strong clinical judgment, excellent communication skills, and a heart for patient-centered care.
What You’ll Do
- Conduct OASIS assessments and initial start-of-care visits to establish individualized plans of care
- Evaluate functional limitations and develop personalized therapy goals and interventions
- Provide lymphedema management and treatment, if certified
- Guide patients through therapeutic exercises and mobility programs
- Educate patients and caregivers on home exercise and safety techniques
- Collaborate with physicians, nurses, and other members of the care team
- Monitor patient progress, update care plans, and maintain thorough documentation
- Ensure compliance with all Medicare and home health standards
What We’re Looking For

Current Florida Physical Therapist (PT) license (required)

OASIS and start-of-care experience (required)

Lymphedema certification (preferred but not required)
- Previous home health experience strongly preferred
- Excellent communication, time management, and documentation skills
- A compassionate, dependable, and professional attitude
