● Provides initial and ongoing interdisciplinary assessment of the patient to identify the patient’s level of functioning, swallowing and communication ability.
● Provides speech therapy in compliance with practitioner orders, patient’s plan of care, FLHC policies and procedures and federal and state guidelines
● Develops a plan of care in consultation with the health care practitioner and other team care members.
● Instructs patients and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
● Instructs patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language and cognition.
● Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age.
● Documents all care and services provided according to professional standards and agency policy and procedures.
● Adheres to all agency policies and procedures.
● Completes all mandatory agency training and in-service activities.
● Participates in the agency’s quality management/QAPI activities.
● Knowledge of speech therapy practice, with emphasis on the provision of physical therapy services in the home environment.
● Ability to foster constructive and therapeutic relationships with patients/clients and their families.
● Ability to communicate effectively, both orally and in writing.
EDUCATION/EXPERIENCE
● Licensed as a Speech Pathologist
● Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association.
● Two years’ experience as a Speech Therapist. Previous experience in the home care or outpatient therapy industry preferred.
CERTIFICATIONS, LICENSURE, & REGISTRATION
● Current, valid license to practice as a Speech Pathologist.
● CPR certification.
● Current, valid driver's license.
● Access to a reliable automobile
