Base Salary: $48,000–$50,000/year + $2,000 differential per additional case (See details below)

ServiceNet’s Shared Living Program is are seeking a dedicated and compassionate Case Manager to coordinate care for individuals living in Shared Living homes. This is a vital role within our team—ensuring each person supported receives the care, dignity, and individualized services they deserve.
Key Responsibilities:
- Manage and oversee the support of individuals in Shared Living homes
- Coordinate and monitor services provided by Care Providers, Individual Support Staff (ISS), Personal Support Staff (PSS), and Respite Providers
- Ensure all services align with funding regulations, agency policies, and quality standards
- Identify stressors and implement interventions to maintain placement stability
- Supervise support staff and provide crisis intervention as needed
- Collaborate with the Department of Developmental Services (DDS) and other agencies
- Complete all required reports and assessments in a timely and professional manner
What You Bring:
- Broad knowledge of human services or social work
- Bachelor's degree in a related field preferred
- 3–5 years of progressively responsible experience
- Strong organizational and communication skills
- Computer proficiency (Microsoft Office and related systems)
- Valid driver’s license and reliable transportation
Salary Details:

Starting Base: $48,000 -$50,000/year based on experience, education, and workload

Differential: $2,000 for each additional case

Minimum Caseload: 15

Maximum Caseload: 20
