Case Manager
Department: Adult Family Care
Location: Springfield, MA
Schedule: Monday-Friday, 9a-5p (with flexibility for program needs)
Base Salary: $50,000–$52,000/year + $1,500 differential per additional case (see details below)

ServiceNet’s Adult Family Care Program is seeking a compassionate and dedicated Case Manager to oversee and coordinate care for individuals living in qualified family settings. In this vital role, you’ll help ensure that every individual we support receives person-centered care in a safe, supportive, and nurturing home environment.
Key Responsibilities:
- Manage and oversee the support of individuals in Adult Family Care placements
- Coordinate and monitor services provided by Care Providers, Primary Support Staff, and Respite Providers
- Ensure all services meet agency standards, MassHealth regulations, and quality expectations
- Identify and respond to stressors and conflicts to maintain placement stability
- Provide crisis intervention services when needed
- Collaborate with MassHealth and other state/private agencies
- Complete required assessments, reports, and documentation in a timely and professional manner
- Supervise direct support staff and oversee care/respite provider performance
What You Bring:
- Broad knowledge of human services or social work
- Bachelor’s degree in a related field required
- 3–5 years of progressively responsible experience
- Excellent organizational, interpersonal, and communication skills
- Working knowledge of computer systems and software (e.g., Microsoft Office)
- Valid driver’s license and reliable transportation

Base Salary: $50,000–$52,000/year (based on experience, education, and workload)

Differential: $1,500 for each additional case above the base caseload of 26

Caseload Range: Minimum 26, Maximum 30
