Job Summary:
As an External Client Care Coordinator with Comfort Keepers, you’re the heartbeat of our in-home care services across Hazleton, Pottsville, Selinsgrove, Cleona, and Bloomsburg. Think of yourself as a navigator, ensuring clients get the care they need while keeping everything running smoothly behind the scenes. You’ll collaborate with our Support Team to coordinate services, manage resources, and connect with staff, families, community providers, and third-party payers.
Essential Functions:
- Be the first point of contact—handling inquiries, qualifying leads, and scheduling visits
- Assess client and caregiver needs, ensuring safe, high-quality care
- Solve scheduling puzzles—coordinate shifts, resolve issues, and check in regularly
- Support caregivers and families through training and guidance
- Keep compliance in check with accurate documentation and monitoring
- Serve as the critical link between clients, providers, and the Client Services team
- Participate in on-call rotations and management meetings to drive continuous improvement
- Handle administrative tasks while bringing innovation to care coordination
