Job Summary:
As an External Coordinator at Comfort Keepers, you’ll ensure continuity of care for clients across Hazleton, Pottsville, Selinsgrove, Cleona, and Bloomsburg. Collaborate with our Support Team to coordinate in-home services, manage client care resources, and connect with staff, families, community providers, and third-party payers.
Essential Functions:
- Handle client inquiries, qualify leads, and schedule visits
- Assess client and caregiver needs, ensuring safety and quality care
- Coordinate shifts, resolve scheduling issues, and conduct regular visits
- Facilitate caregiver and family training
- Maintain compliance through documentation and care monitoring
- Serve as a vital link between clients, providers, and the Client Services team
- Participate in on-call rotations and management meetings
- Complete administrative tasks and drive care improvements

- Proficiency in Microsoft Office and related software
