Position Summary:
a2z Physical Therapy is seeking a skilled and compassionate Nurse Practitioner (NP) to provide in-home care with a focus on pain management and orthopedic conditions. This role involves delivering high-quality clinical care, managing chronic and acute pain, and coordinating comprehensive treatment plans tailored to each patient's orthopedic and rehabilitation needs. The ideal candidate excels in interdisciplinary collaboration, patient education, and independent clinical decision-making.
We are currently hiring for PRN or Part-time positions, with the potential for transition to Full-time based on performance and program expansion. This is an excellent opportunity for growth within a supportive and evolving clinical team.
Key Responsibilities:
- Perform in-home health assessments and focused exams, especially for musculoskeletal and orthopedic conditions.
- Develop and manage individualized treatment plans for patients dealing with chronic pain, post-operative recovery, or orthopedic injuries.
- Collaborate and communicate regularly with physical therapists to align goals and care plans.
- Coordinate referrals to specialists, radiology, and additional services as needed.
- Manage medications, including pain management prescriptions and controlled substances, with attention to safety and compliance.
- Provide patient and caregiver education on orthopedic care, pain management strategies, and medication adherence.
- Adjust treatment plans based on patient progress, outcomes, and evolving needs.
- Maintain accurate and timely documentation in the EMR system.
- Utilize telehealth technologies as needed for follow-ups and consultations.
Required Qualifications:
- Licensed Nurse Practitioner (NP) in [State], with current prescriptive authority.
- 1–2+ years of experience in pain management, orthopedics, or related specialties.
- Experience in home health is preferred.
- Knowledge of orthopedic injuries, post-op recovery protocols, and chronic musculoskeletal pain.
- Certification in Pain Management (AANP/ANCC/ACNPs or equivalent) is a plus.
- Valid driver’s license and reliable transportation.

This goal is the driving force behind every step of the company, from the clinicians in the field doing the treatments to the management and marketing team.
We understand that this is a difficult time for our patients, so not only do we want to help them recover from their physical ailments, but we also want to help them with understanding the steps that come along with managing a personal injury. We do not offer the typical treatments that you find from most providers after getting into a car accident. Our patients will each receive one-on-one care directly from a physical therapist during their entire treatment, and manual (hands-on) treatment is performed at each visit.
We make sure the patient receives the skilled care they need, and deserve, based on the specific injury and deficit reported by the patient directly.
