Superior Home Care and Hospice - Salt Lake City, UT
We are a locally owned and operated hospice and home health agency, celebrating our 27th year. We have an excellent reputation for providing very high quality, patient-focused care. We are looking for a qualified Hospice and Home Health RN Case that shares our values and wants to assist in our mission. Your duties will include but not be limited to:
Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.
Create patient centered goals with patients, family members and providers
Assess, supervise and direct patient care
Develop plans of treatment
Ensure chart compliance with state regulations
Coordinate patient care with physicians and other providers
Hospice and Home Health are both challenging and rewarding! This job requires a person who will never forget our work requires a team effort. Other requirements include:
Licensed as a Registered Nurse by the State of Utah
Current Utah driver's license and automobile insurance
CPR certification
A minimum of 1 year nursing experience is required
Hospice and home health experience is preferred but not required
