IJN Health Systems, LLC, a Skilled Home Health Agency in Newark, Delaware, is looking for a Physical Therapist to join our strong compassion and provide rehabilitation services. Our ideal candidate is a self-starter who is motivated, engaged, and dependable.

Physical Therapist (PT) – Home Health Care, PRN or Part-Time


Position Summary:

  • Provide services to treat impairments, functional limitations, disabilities, or changes in physical function and health status for patients normally performed in their home.
  • Plan, organize and implement physical therapy services within the confines of state licensing law to individuals, families, and groups in accordance with agency policies, procedures, processes, and agency code of ethics. 
Qualifications / Requirements 

  • Bachelor's degree, Master's level, or DPT, which meets the requirements of an approved physical therapy program, and is licensed through examination by the State Board of Physical Therapy.
  • Graduate of a Physical Therapy education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or APTA.
  • One year of Physical Therapy experience is required.
  • 1-2  years of Home Health experience is preferred.
  • Competency in computer skills.
  • Proficient in OASIS , SOC
  • CPR certified
  • Experience with EMR systems
  • Pass a background check
  • Provide two (2) professional references
  • Able to travel to cases as assigned
  • Ability to work independently and manage time effectively
  • Meets the requirements of federal and/or state criminal and abuse background checks
  • Must have access to dependable transportation, auto insurance coverage
