Capital Home Health Care LLC is a small, growing company committed to providing exceptional care for individuals with developmental disabilities. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team on a part-time basis!

Responsibilities:
- Provide high-quality nursing care to clients in accordance with their care plans
- Administer medications and treatments as prescribed
- Monitor and document client health status and progress
- Collaborate with other healthcare professionals and support staff to ensure comprehensive care
- Educate clients and their families on health management and preventive care
- Ensure compliance with all health and safety regulations

Who We Serve
CHHC proudly offers 24/7 care to clients across Burlington, Camden, Mercer, and Middlesex Counties. We offer Medicaid-approved services to individuals with developmental disabilities, including those with medical comorbidities. We accept a range of insurance plans, including Medicaid, Horizon NJ Health, Amerigroup, and private pay options to ensure accessibility to quality care.
