💼 Your Role

As a Licensed Medical Social Worker, you will be an essential advocate for our patients and their families — providing emotional support, coordinating care, and ensuring that every individual receives the highest standard of service and compassion.

🩺 What You’ll Do

  • Provide professional medical social work services in alignment with hospice standards of care.

  • Conduct psychosocial assessments to understand each patient’s emotional and social needs.

  • Collaborate closely with physicians, nurses, and families to create individualized care plans.

  • Make follow-up visits to evaluate progress and adjust care strategies as needed.

  • Develop interventions tailored to patients’ evolving needs and circumstances.

  • Maintain accurate, timely, and confidential documentation.

🎓 What You Bring

  • Education: Master’s degree in Social Work (MSW)

  • Licensure: Michigan LMSW or LLMSW accepted

  • Experience: Minimum one year in healthcare, hospice, or related setting

  • Certifications: Current CPR certification and valid driver’s license

  • Bonus: Psychiatric or behavioral health experience preferred

Compensation: $72,000.00 - $75,000.00 per year
