Position Overview

We are seeking a compassionate and experienced Registered Nurse to lead our home health clinical operations in Cerritos, CA. The Home Health Clinical Manager will oversee a multidisciplinary team, ensuring the delivery of high-quality, patient-centered care in compliance with all regulatory standards. This role combines clinical leadership with operational oversight, including staff management, quality assurance, and regulatory compliance.

Key Responsibilities

  • Clinical Oversight: Develop, implement, and evaluate individualized patient care plans in collaboration with physicians and the care team.

  • Staff Management: Recruit, train, supervise, and evaluate clinical staff, including RNs, LVNs, and CNAs. Provide ongoing education and performance feedback.

  • Regulatory Compliance: Ensure adherence to federal, state, and local regulations, including Medicare Conditions of Participation and CHAP/ACHC standards.

  • Quality Assurance: Monitor and improve clinical outcomes through performance improvement initiatives and regular audits.

  • Case Management: Oversee patient admissions, re-certifications, and discharges. Manage complex cases and coordinate interdisciplinary care.

  • Operational Leadership: Assist in budgeting, scheduling, and resource allocation to optimize efficiency and service delivery.

  • Community Relations: Build and maintain relationships with referral sources, physicians, and community organizations to enhance service delivery and patient satisfaction.

Qualifications

  • Education: Graduate of an accredited School of Nursing.

  • Licensure: Active RN license in California.

  • Experience: Minimum of 3 years of nursing experience, with at least 1 year in a home health or hospice setting. Previous management or supervisory experience preferred.

  • Skills: Strong leadership, organizational, and communication skills. Proficiency in electronic medical records (EMR) and knowledge of OASIS documentation.

  • Certifications: Current CPR certification.

Compensation and Benefits

Compensation: $100,000.00 - $120,000.00 per year
