General Summary
The LVN Team Lead provides patient care, performs patient and community education activities, and ensures all patient information is kept secure and confidential. Supervises and provides leadership to the assigned practice, including but not limited to promoting high standards of care through the implementation of policies, protocols, and practice guidelines. Must have excellent communication skills and work well with others.
Primary Responsibilities
1. Provides patient care under the supervision of the Health Center’s clinician and documents all services in the Electronic Health Record (EHR).
2. Performs one-to-one patient education on disease processes, diet and medication management as ordered by the health center clinician.
3. Performs venipuncture to obtain laboratory specimens and ensures proper labeling and handling of specimens.
4. Assists clinician with processing medication refills upon request and obtains prior authorizations for referrals and diagnostic testing ordered by clinician.
5. Assists staff in keeping the examination and laboratory rooms clean and well stocked at all times.
6. Responsible for reviewing and verifying all Title XIX and Home Health orders are completely and accurately filled out.
7. Responsible for managing daily clinical activities including clinical staffing, timely patient follow-up, and minimizing exam room waiting time.
8. Maintains adequate inventory of supplies and ensures all equipment is properly functioning.
9. Collaborates with medical office manager to optimize clinician schedules and ensure appointment slots are filled.
10. Responsible for overview and administration of the Texas Vaccine for Children (TVFC) and Adult Safety Net (ASN) Program.
11. Ensures patient concerns are addressed in a timely manner and according to policies and procedures.
12. Reviews and approves clinical employees' timesheets.
13. Maintains clean and safe facilities that meet all local, state, and federal requirements.
14. Participates in community activities and events to promote health center services.
15. Other duties as assigned by the supervisor.
Work Experience
Required Preferred
● Must have medical office (private physician or public health clinic) experience Education/Certifications/Licensure

● Must be a graduate of an accredited nursing school.
● Licensed in the State of Texas without restrictions

● Must have knowledge of medical terminology.

Physical Requirements
Light to medium physical duties include standing, walking, reaching, bending, and talking for up to eight (8) hours a day. Must have good vision. Must be able to lift thirty-five pounds. Lifting will be primarily confined to occasional on-premises lifting of boxes of office supplies and minor office equipment. Non-physical demands include performing multiple tasks simultaneously, judgement decisions and working closely with others as part of a team. Must have a valid driver’s license and a safe driving record and be able to pass a pre-employment physical and criminal history background check.
