Licensed Vocational Nurse (LVN)

Job Summary:
The LVN provides direct nursing care to medically fragile residents in a CLHF setting. Under the supervision of an RN or physician, the LVN ensures high-quality, compassionate care tailored to each patient’s clinical needs.
Key Responsibilities:
- Administer prescribed medications and treatments accurately.
- Provide skilled care for residents with tracheostomy, ventilator dependence, G-tubes, and wound care needs.
- Monitor and record vital signs, intake/output, and patient status changes.
- Communicate with an interdisciplinary team regarding resident conditions and care plans.
- Document all care provided in accordance with state regulations and facility policies.
- Respond promptly to patient needs and emergencies.
- Assist with ADLs (activities of daily living) when necessary in coordination with CNAs.
- Support infection control and safety protocols.
Requirements:
- Current LVN license in the state of California.
- Valid BLS/CPR certification.
- Previous experience in a subacute, CLHF, or skilled nursing environment preferred.
- Familiarity with ventilator and trach care is highly desirable.
- Strong documentation and communication skills.
- Ability to work independently and collaboratively in a small team setting.
- Physical ability to perform nursing duties, including lifting, bending, and long periods of standing.
Shift & Pay:
- Shifts: 12-hour shifts, the rate is based on competitive compensation, depending on experience.
Compensation: $33.00 - $36.00 per hour
