Position Summary:
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to provide in-home nursing care to patients under the supervision of a Registered Nurse (RN). The ideal candidate has strong clinical and interpersonal skills and is committed to supporting patients' health, independence, and well-being.

Key Responsibilities:
- Provide direct patient care in the home according to the established plan of care.
- Perform skilled nursing procedures such as wound care, medication administration, injections, and catheter care.
- Monitor and document patients' condition and report any changes to the supervising RN or physician.
- Educate patients and family members on disease process, medications, and care needs.
- Collaborate with the interdisciplinary team to support the patient’s recovery and goals.
- Maintain accurate, timely, and thorough documentation in compliance with state, federal, and agency guidelines.
- Follow infection control and safety protocols at all times.
Qualifications:
- Current and valid LVN license in the state of [Insert State].
- Current CPR/BLS certification.
- Minimum of [1 year preferred] nursing experience (home health experience a plus).
- Valid driver’s license and reliable transportation.
- Strong communication, organizational, and critical thinking skills.
- Ability to work independently in a home setting.
Compensation: $50.00 - $50.00 per hour
