Job Summary:

We are seeking a compassionate and licensed Physical Therapist (PT) to join our team. The ideal candidate will assess patients, develop individualized treatment plans, and implement physical therapy interventions to help patients improve mobility, manage pain, and restore physical function. This role involves working closely with physicians, patients, and families to ensure high-quality care and optimal recovery outcomes.

Key Responsibilities:

  • Perform thorough evaluations of patients’ physical abilities and limitations.
  • Develop and implement customized treatment plans based on diagnoses and patient goals.
  • Use therapeutic exercises, hands-on therapy, and equipment to aid patient recovery.
  • Educate patients and their families on home exercises and prevention strategies.
  • Monitor patient progress, adjust treatments as needed, and document all findings in medical records.
  • Collaborate with physicians and other healthcare team members for coordinated care.
  • Maintain compliance with federal, state, and professional regulatory standards.

Requirements:

  • EMR HomeCare Home Base (HCHB) experience preferred. Home Health experience including OASIS required.
  • 2 years experience in a healthcare setting required -- Community/home health experience is
  • preferred.
  • Required certs: BLS & NPI 
  • Also needs a CA PT license
  • HH requires a current driver's license & proof of automobile insurance.
  • Good oral and written communication skills; has good organizational skills.
  • Provides physical therapy services to patients/clients according to a written physician's plan of care. 
  • Assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
  • Evaluates home environment. Assesses for muscle strength, mobility, gait, ROM - potential for rehab. Directing physical therapy treatment.
  • Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy.
  • Reporting the patient's/client's reaction to treatment or changes in condition to the physician.

Compensation: $50.00 - $55.00 per hour
