Position Summary:
St. Thomas Community Health Center is seeking a dedicated and proactive Community Clinic Nurse (LPN or RN) for our Gretna-based Westbank clinic. This onsite role provides direct, face-to-face support for patients in collaboration with providers, helps connect patients to chronic care services, and supports the daily function and leadership of the medical assistant team. The Nurse plays a vital role in patient education, provider support, and care coordination to ensure our patients receive timely, high-quality, and equitable care.
Key Responsibilities:
Care Coordination & Patient Support/Education
- Collaborates with providers and care teams to identify patients appropriate for care coordination and ensures proper referrals are initiated particularly to health coaches for chronic condition support.
- Engages patients in understanding and managing their health conditions through education on clinical devices (e.g., BP cuffs, glucometers, continuous glucose monitors) and medications, including use of injectables such as insulin.
- Reinforces medication adherence strategies and offers guidances.
Population Health & Preventive Outreach
- Supports outreach to patients due for preventive and follow-up care using available data sources and clinic tools.
- Contributes to efforts that improve care gap closure and enhance overall patient outcomes through preventive and chronic disease services and medication usage as part of ongoing care support.
Team Leadership & Supervision
- Serves as the on-site lead for medical assistants, offering general supervision, workflow guidance, and day-to-day support.
- Participates in the onboarding and training of new medical assistants.
- Assists in ensuring clinical protocols are followed and contributes to quality improvement efforts within the nursing team.
Provider Support & Clinic Operations
- Works closely with providers to support clinic flow and visit efficiency, ensuring patients are prepared and ready for their appointments.
- Helps identify and address clinical needs that support providers, including coordination of follow-up appointments, immunization needs, and preventive screenings.
- Assists in maintaining consistent and balanced schedules across the team by recognizing and filling open appointment slots appropriately.
Clinical Support & Vaccine Oversight
- Monitors vaccine management; able to assist with administration and related support tasks when needed.
- Ensures provider and team awareness of available vaccines and coordinates timely patient access.
Other Duties
- Attends nursing and general staff meetings as scheduled.
- Supports the policies and procedures of St. Thomas Community Health Centers and assists in policy review as requested.
- Demonstrates consistent and timely attendance
- Promotes positive and productive communication with clinic staff.
- Utilizes established communication channels and appropriate lines of authority for problem-solving.
- Assists in documentation of incidents and participates in safety and emergency preparedness drills.
- Performs other duties as requested by the Chief Nursing Officer and Medical Director.
- Maintains HIPAA compliance.
Qualifications:
- Current LPN or RN license in the State of Louisiana.
- Experience in community health, chronic disease management, or outpatient settings preferred.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a fast-paced clinical environment.
- Bilingual skills (English/Spanish) a plus.
- Familiarity with EPIC EHR a plus
