Work Schedule: Tuesday-Wednesday-Friday
Pay Rate: $41–$43 per hour
Estimated Annual Salary: $68,224–$89,440, based on weekly hours worked

Responsibilities of an RN Case Manager:
- Provides care as defined within the scope of the state’s nurse practice act.
- Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs.
- Prepares clinical and progress notes and summaries of care.
- Serve as backup for Critical Care Clients, In-School Staffing & PDN
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
- Helps educate and coach care-giving staff and assists with new hire clinical training, as needed.
- No OASIS work.
RN Case Manager Requirements:
- Current unrestricted RN license in Minnesota
- Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred.
- Critical Care Experience (1-2 years)
- Active CPR Certification
- Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)
- Reliable car & Auto Insurance
- Valid Driver's License
- Clean background and Criminal Record
- Understands regulatory compliance with state and federal regulations.
- Ability to work on-call rotation with compensation
