Position Overview:
We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated Socorro team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care, including symptom and medication management.
- Coordinate resources, referrals, and care services for patients and families.
- Offer education and emotional support to patients and their loved ones regarding end-of-life care and planning.
- Maintain accurate and timely clinical documentation in accordance with hospice policies, Medicare/Medicaid guidelines, and applicable regulations.
- Ensure compliance with HIPAA, state/federal healthcare laws, and organizational procedures.
- Supervise and provide direction to Licensed Vocational Nurses (LVNs) and Home Health Aides (HCAs) as needed.
- Participate in interdisciplinary case conferences, quality assurance activities, and continuing education.
- Participate in scheduled home visits and on-call rotation to ensure 24/7 patient support.
Qualifications:
- Graduate of an accredited school of nursing.
- Current and valid RN license in the state of New Mexico.
- At least 1 year of nursing experience; hospice or end-of-life care preferred.
- Strong critical thinking, organizational, and assessment skills.
- Compassionate and empathetic approach to patient care.
- Excellent verbal and written communication skills.
- Valid driver's license and current automobile liability insurance.
- Reliable transportation for patient visits in home and community settings.
