As a Hospice RN, you can expect:
As a Hospice RN, you can expect:
- Currently looking to fill our Macomb and Eastern Wayne territory
- Its a salaried position, working from 8:00 am - 5 pm Monday through Friday.
- Mileage Reimbursement
- Completing initial evaluation and re-evaluating the patient's nursing needs during each visit
- Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing and evaluating the patient plan of care.
- Deliver high quality nursing care to hospice patients in their homes
- Monitor patient conditions and respond proactively to changes in status
- Collaborate with physicians and interdisciplinary team members to coordinate care
- Educate patients and families on disease progression, medication, and comfort care
- Support families emotionally and practically through end of life care
- Ensure compliance with physician orders and regulatory standards
What You Bring:
- Minimum two years experience as a Registered Nurse
- Strong Hospice/Home Care background preferred
- Current RN license in the State of Michigan
