We are looking for a dedicated Registered Nurse (RN) Case Manager to become an essential part of our hospice care team. In this role, you’ll help ensure each patient’s journey is met with compassion, skill, and tailored support. In this role, you’ll help ensure each patient’s journey is met with compassion, skill, and tailored support. Key Responsibilities:
- Perform comprehensive evaluations of patients’ physical, emotional, and psychosocial needs
- Develop and carry out individualized care plans in partnership with the interdisciplinary team
- Deliver hands-on clinical care, including pain management and medication administration
- Facilitate coordination of services, supplies, and external resources for patients and families
- Provide education, guidance, and emotional support throughout the hospice journey
What You Bring:
- Active New Mexico RN license (required)
- Previous experience in hospice, palliative care, or similar settings is a plus
- Excellent assessment and clinical judgment skills
- A warm, empathetic approach to working with patients and families
- Ability to collaborate effectively with team members across disciplines
