We are currently looking for a Part-Time Medical Social Worker (MSW) to help serve patients in the Gallup area. If you are someone who finds purpose in guiding others through emotional and practical challenges associated with end-of-life care, we’d be honored to have you on our team.
Role Overview:
As a Hospice Social Worker, you will play a vital role in supporting patients and families through emotional, social, and financial transitions. Working closely with our interdisciplinary team, you will ensure each patient receives compassionate care that addresses their full spectrum of needs.

Key Responsibilities:
- Respond to referrals from the clinical team for social work support
- Perform in-depth assessments to identify emotional, social, and economic concerns
- Offer brief counseling and assist families with navigating available resources
- Assist with planning, goal-setting, and connection to community-based services
- Keep accurate records and maintain documentation in accordance with care plans
- Participate in case discussions and contribute to interdisciplinary planning
- Provide home visits and be available for on-call duties as required
- Maintain confidentiality while preserving patient dignity throughout care
Required Qualifications:
Education:
- Master’s in Social Work (MSW) from a CSWE-accredited program
Experience:
- Minimum of one year in a social work role; hospice or healthcare experience preferred
Licenses & Certifications:
- Licensed to practice Social Work in New Mexico
- Current CPR certification
- Valid driver’s license and reliable transportation with auto insurance
Skills & Traits:
- Excellent communication and time management
- Deep empathy and a calm, professional demeanor
- Familiarity with Medicare/Medicaid and community resource networks
- Ability to work independently in home-based environments
