Responsibilities:
- Provide direct patient care, including administering medications and monitoring vital signs.
- Assist with daily living activities and ensure patient comfort and safety.
- Collaborate with healthcare teams to develop and implement personalized care plans.
- Educate patients and families on health management and wellness strategies.
- Document patient progress and report any changes in condition to the supervising nurse.
- Maintain compliance with healthcare regulations and company policies.
- Participate in ongoing training and professional development opportunities.
- Build strong relationships with patients and their families to foster trust and support.
Requirements:
- Current LVN license in the state of California.
- Minimum of 1 year of nursing experience, preferably in home health or hospice care.
- Strong clinical assessment skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate demeanor with a commitment to patient-centered care.
- Basic Life Support (BLS) certification required.
- Reliable transportation and valid driver’s license.

Compensation: $50.00 - $70.00 per hour
