Clinical/Consultant Pharmacist
Long Term Care
Primary Objective:
We are seeking a dedicated and knowledgeable long-term care clinical pharmacist to join our team. This role involves providing expert pharmaceutical care to residents in long-term care facilities, ensuring compliance with regulatory standards, optimizing medication therapy, collaborating with healthcare providers to improve patient outcomes as well as supporting our clinical MTM services in long term care.
Key Responsibilities:
- Conduct comprehensive medication regimen reviews for residents in long-term care settings.
- Identify and resolve drug-related problems, including adverse drug reactions, drug interactions, and therapeutic duplications.
- Provide recommendations to physicians, nurses, and facility staff to optimize pharmacotherapy.
- Ensure compliance with federal and state regulations, including CMS and state board of pharmacy requirements.
- Educate facility staff on medication-related topics and best practices.
- Maintain accurate documentation of clinical interventions and consultations.
- Participate in quality assurance and performance improvement initiatives.
- Support clinical initiatives in the long-term care space such as chronic care management, comprehensive medication reviews and other medication management services.
- Collaborate with pharmacy operations and clinical services to support our long-term care initiatives.
- Some travel is required, up to 10% of the time.
Knowledge, skills and abilities:
• Bachelor of Science in Pharmacy (required); Doctor of Pharmacy (preferred); Board Certified in Geriatric Pharmacy (preferred).
• Active pharmacist license in the state(s) of practice. Multi-state licensure preferred. Our territory includes: Minnesota, North Dakota, Wisconsin, Iowa, South Dakota.
• Prior experience in long-term care or geriatric pharmacy preferred.
• Strong clinical knowledge and understanding of geriatric pharmacotherapy.
• Excellent written and oral communication skills. Excellent organizational and problem-solving skills.
• Ability to work independently and meet required deadlines with little oversight.
• Ability to travel to assigned facilities as needed. The exact territory of this position has not yet been defined.
• Strong time management skills and flexibility are essential.
