Position Overview 
The Intake and Community Outreach Coordinator plays a dual role in supporting the 
agency’s mission. This individual is responsible for completing intake packets, scheduling 
clinical assessments, and coordinating with the Clinical Supervisor to ensure new clients 
transition smoothly into services. In addition, the Coordinator will represent the agency in 
the community, building referral relationships and strengthening partnerships with schools, 
treatment centers, foster care agencies, and other providers. 
Key Responsibilities 
- Complete intake packets for referred clients accurately and in a timely manner. 
- Schedule assessments with the Clinical Supervisor and ensure clients and families are 
informed of appointment times. 
- Conduct outreach to schools, community organizations, residential treatment centers, 
foster care agencies, hospitals, and other referral sources. 
- Develop and maintain strong community partnerships to increase client referrals. 
- Attend and represent the agency at outreach events, presentations, and community 
meetings. 
- Submit outreach documentation, including Event Summary Reports and Referral Source 
Forms, within required timelines. 
- Maintain accurate records in CRM/spreadsheets of contacts, outreach efforts, and 
outcomes. 
- Meet weekly outreach goals, including verified in-person visits and follow-up. 
- Collaborate closely with the Clinical Supervisor and administrative team to ensure smooth 
intake and service coordination. 
Qualifications 
- High school diploma or equivalent required; associate’s or bachelor’s degree in a 
behavioral health or human services-related field preferred. 

 
- Previous experience in community outreach, intake coordination, case management, or 
client services strongly preferred. 
- Strong organizational skills with attention to detail and accuracy. 
- Excellent communication, networking, and interpersonal skills. 
- Ability to work independently, manage time effectively, and meet deadlines. 
- Comfortable engaging with diverse community partners and client populations. 
- Proficient with technology tools such as spreadsheets, CRM systems, and email 
communication. 
- Must have reliable transportation for community outreach activities.
