About the Role

We are seeking a dedicated and driven Care Coordinator to join our Santa Cruz team. This role is ideal for someone who thrives in a Care Manager capacity while also empowering their team to succeed. Care Coordinators are the bridge between clients, community resources, and internal support — ensuring no client falls through the cracks.

Key Responsibilities

  • Act as primary point of contact for assigned clients, ensuring needs are met and progress is tracked.

  • Support populations of focus, including:
     • Pregnant and postpartum individuals
     • People experiencing homelessness
     • Justice-involved individuals
     • Members with complex health and social needs

  • Conduct regular check-ins with clients to assess needs and provide linkage to resources.

  • Collaborate with other coordinators, supervisors, and community partners to achieve the best outcomes.

  • Document client interactions accurately and in compliance with program standards.

  • Balance a caseload while helping team members navigate challenges.

Qualifications

  • Must live in Santa Cruz County.

  • Strong interpersonal and organizational skills.

  • Experience working with vulnerable populations preferred.

  • Ability to work independently while supporting team goals.

  • Driven, compassionate, and adaptable to client needs.

This is a remote position.

Compensation: $17.50 - $25.00 per hour
