Meridian Home Health, a Specialized Home Health Agency, equal opportunity employer, seeks exceptional, qualified Physical Therapists to service its Skilled Intermittent Home Health patients in Alameda and its surrounding
We are looking for a committed Physical Therapist to act as the patient’s partner throughout the treatment of restoring motion and ensuring that the client will function at their personal best. You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.
Job Description:
- Provide physical therapy services to patients according to written physician's plan of care.
- Initiate physical therapy program and instructs other personnel/ or family/ caregiver members in certain phases of physical therapy.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Participates in discharge planning for patient
- Provide accurate and timely documentation
- Participates in coordination of home health services with team
- Supervises Physical Therapist Assistants according to organization policy and state regulations
License/Certification:
- Current PT License
- Current BLS/CPR Certification
- Current CA Driver's License
Qualifications:
- Excellent communication and interpersonal skills
- The ability to create a positive, nurturing environment
- The ability to work well and make decisions in a high-stress environment
- Quick submission of timely and correct documentation
