We are seeking a highly organized and detail-oriented Intake Coordinator to manage the client onboarding process from initial inquiry through admission to services. This role serves as the first point of contact for prospective clients, referral sources, and family members. The Intake Coordinator ensures a smooth, compliant, and efficient intake experience while supporting agency growth and maintaining regulatory standards.

This position requires professionalism, strong communication skills, urgency, and the ability to manage multiple priorities in a fast-paced home health environment.
Key Responsibilities

  • Respond promptly to new client inquiries via phone, email, and referral portals

  • Collect and verify required intake documentation (demographics, insurance information, physician orders, authorizations, etc.)

  • Coordinate initial assessments and communicate scheduling with clinical staff

  • Verify insurance eligibility and benefits when applicable

  • Enter client data accurately into EMR/EVV systems

  • Ensure all intake documentation meets state, payer, and company compliance standards

  • Maintain communication with referral sources, case managers, and families

  • Track referral conversion metrics and maintain intake logs

  • Follow up on pending documentation and authorizations to avoid delays in start of care

  • Provide administrative support related to admissions and census tracking
    Qualifications

    • High school diploma required

    • Previous experience in home health, healthcare administration, or medical intake preferred

    • Knowledge of insurance verification and authorization processes

    • Familiarity with EMR systems and electronic documentation

    • Strong organizational and multitasking skills

    • Excellent written and verbal communication skills

    • Professional phone etiquette

    • Ability to maintain confidentiality and handle sensitive information

Schedule

Monday–Friday, standard business hours (with occasional flexibility as needed).
