Key Responsibilities
- Perform comprehensive initial evaluations and re-assessments to determine patients’ mobility, balance, and functional needs.
- Develop and implement customized treatment plans aligned with patient goals and care plans.
- Deliver gait training, range-of-motion exercises, strengthening, and fall prevention strategies.
- Educate patients and caregivers on home safety, adaptive techniques, and proper use of assistive equipment.
- Collaborate closely with PTAs, RNs, OTs, and interdisciplinary team members to ensure coordinated, high-quality care.
- Accurately complete documentation in the EMR system per Medicare and agency guidelines.
- Utilize OASIS for documentation and assessment accuracy; prior experience with OASIS is required.
- Experience with EMR systems such as Kinnser (WellSky), or similar platforms is preferred.
- Monitor patient progress, communicate changes in condition to the care team, and adjust care plans as needed.
- Supervise PTAs and aides in accordance with state and federal regulations.
Qualifications
- Current California Physical Therapist (PT) license.
- Minimum of 1 year of experience in a Home Health setting, including use of OASIS documentation.
- Proficiency with EMR systems (WellSky/Kinnser preferred).
- Strong interpersonal and communication skills.
- Ability to work independently while maintaining strong collaboration with interdisciplinary teams.
- Valid driver’s license and reliable transportation.
Compensation: $60.00 - $70.00 per hour
