🔹 Reinforce and support hospice-directed plans of care
🔹 Observe and communicate changes in patient condition to hospice teams
🔹 Serve as a liaison between patients, families, caregivers, and leadership
🔹 Respond to questions with empathy, clarity, and professionalism
🔹 Maintain accurate documentation and support compliance standards
🔹 Reinforce and support hospice-directed plans of care
