About the Role:
As a Care Coordinator at Assisting Hands Home Care North & South Charlotte, you will play a vital role in enhancing the lives of our clients through personalized care management. Join our passionate team in Charlotte, NC, and make a meaningful impact in the community while fostering relationships with clients and caregivers. The right candidate must enjoy working with seniors and adults needing home care. 


Responsibilities:
  • Coordinate and oversee client care plans to ensure quality service delivery.
  • Conduct assessments and evaluations to determine client needs and preferences.
  • Communicate effectively with clients, families, and caregivers regarding care updates.
  • Schedule and manage caregiver assignments to ensure optimal client support.
  • Monitor client progress, adjust care plans as necessary and report changes to our RN. 
  • Provide training and support to caregivers to enhance service quality.
  • Maintain accurate and up-to-date client records and documentation.
  • Assist with marketing and community outreach initiatives to promote services.
Requirements:
  • Previous experience in healthcare or home care coordination preferred.
  • Strong communication and interpersonal skills to foster client relationships.
  • Ability to multitask and prioritize in a fast-paced environment.
  • Proficient in using electronic health records and scheduling software.
  • Compassionate demeanor and a commitment to client-centered care.
  • Valid driver's license and reliable transportation required.
  • Willingness to work flexible hours to meet client needs.
  • Active NC CNA license.

Compensation: $18.00 - $23.00 per hour
