Job Description
As a Physical Therapist, you will:
- Provide one-on-one physical therapy services in patients' homes
- Assess and develop individualized treatment plans
- Monitor patient progress and modify plans as needed
- Collaborate with interdisciplinary teams when required
Qualifications
- Doctorate or Degree in Physical Therapy (DPT or equivalent)
- Minimum 2 years of experience in physical therapy (Home Health experience preferred)
- Strong communication and organizational skills
- Self-motivated with the ability to work independently and manage a flexible caseload
- Valid California Physical Therapist License
- Valid California Driver’s License
- Current CPR certification, TB Test, and Professional Liability Insurance
- Pre-employment physical and TB test (within the last 6 months)

We offer our clients stand alone Mobile services, stand alone Telehealth services, as well as, a combination package of Mobile and Telehealth services, when it is determined at the client’s initial consultation that is important for the achievement of the client’s goals.
