Legacy Care is seeking an outgoing, relationship-driven Community Liaison to represent our brand throughout San Diego North County and coastal cities. This role is focused on building strong partnerships with referral sources in the community and within hospital settings. The Community Liaison plays a vital role in increasing brand awareness, developing referral relationships, and promoting a positive image of Legacy Care.
This is a field-based position that requires extensive community outreach, goal-setting, and self-motivation. The ideal candidate will be skilled in referral development, marketing, and sales, and will be able to work independently while effectively communicating the value of our services.
Key Responsibilities:
- Schedule and conduct weekly visits with 20–30 referral sources to maintain visibility and nurture relationships.
- Organize and deliver a minimum of two monthly educational presentations or information sessions to community groups and referral networks.
- Assist Legacy Care in identifying, developing, and maintaining referral relationships with healthcare professionals, discharge planners, social workers, and other community contacts.
- Respond promptly and professionally to all referral source interactions with appropriate follow-up and communication.
- Proactively research and target new opportunities for client referrals within the designated territory.
- Maintain accurate records and submit weekly networking activity reports and progress updates to the Executive Director.
- Monitor trends and provide insight to management on changes in the market, referral activity, and competitor presence.
- Stay informed about industry trends, community events, and key developments affecting referral patterns.
- Actively participate in internal and external professional development opportunities to enhance performance and effectiveness
Candidate Requirements:
- Strong interpersonal, networking, and public speaking skills.
- Excellent verbal and written communication and active listening skills.
- High level of confidence, initiative, and motivation to succeed.
- Ability to manage time effectively, prioritize tasks, and work independently.
- A collaborative mindset with a passion for helping others and strengthening community connections.
- Prior experience in hospital, senior care, or home care settings is preferred.
- Marketing and sales experience is strongly preferred.

Knowledge of the local healthcare landscape—including senior living communities, hospitals, skilled nursing facilities, and physician offices—is a significant plus. They are driven by results, understand the sales funnel, and are comfortable working toward monthly goals with minimal supervision.

Compensation: $65.00 - $70.00 per year
