โญ REQUIRED Experience:
โœ” Willingness to provide approximately 12 hours per week of direct client care
โœ” Strong organizational and problem-solving skills
โœ” Ability to manage multiple staffing priorities in a fast-paced environment
โœ” Excellent communication and relationship-building skills

โญ WITH:
โœ… Active STNA, CPR certification, and current TB test
๐Ÿš— Valid driverโ€™s license with reliable, insured transportation

โญ PREFERRED Experience:
โœ” Social Worker or Nursing degree is a plus
โœ” Home care scheduling or caregiver staffing experience
โœ” Experience with AxisCare or similar home care software
โœ” Background in home care, hospice, assisted living, or healthcare operations
โœ” Experience as a Social Worker, Case Manager, Care Coordinator, Discharge Planner, or Patient Advocate

๐Ÿ“… Caregiver Scheduling & Staffing Coordination:
โœ” Manage caregiver schedules using AxisCare software
โœ” Ensure full client coverage and prevent missed shifts
โœ” Respond to call-offs and urgent staffing needs
โœ” Maintain a dependable backup caregiver pool
โœ” Monitor overtime and scheduling efficiency

๐Ÿค Client Care Coordination:
โœ” Match caregivers with clients based on skills, personality, and care needs
โœ” Understand client care plans and home environments
โœ” Promote client satisfaction and caregiver retention
โœ” Act as a liaison between clients, families, caregivers, and healthcare providers

๐Ÿง‘โ€โš•๏ธ Caregiver Support & Field Operations:
โœ” Address caregiver and client concerns
โœ” Resolve scheduling and care-related issues
โœ” Provide field support to caregivers
โœ” Maintain strong client relationships and continuity of care

๐Ÿ“š Staffing & Caregiver Development:
โœ” Identify caregiver training needs
โœ” Coordinate training with the Hospice Support Nurse
โœ” Recognize staffing gaps and assist with hiring needs

๐ŸŒฟ Hospice Coordination:
โœ” Assign appropriately trained caregivers to hospice clients
โœ” Coordinate with hospice partners as needed

๐Ÿš— Field Engagement (Client Care):
โœ” Provide approximately 12 hours per week of direct client care
โœ” Support clients and caregivers in the field
โœ” Assist with coverage when needed

๐Ÿ“‹ Compliance & Documentation:
โœ” Ensure caregiver documentation meets required standards
โœ” Confirm documentation is uploaded in AxisCare
โœ” Maintain complete caregiver files
โœ” Support compliance with Ohio non-medical home care regulations

โฐ Schedule:

FULL-TIME position combining:

โœ” Caregiver scheduling
โœ” Approximately 12 hours per week of direct client care
