โญ REQUIRED Experience:
โ Willingness to provide approximately 12 hours per week of direct client care
โ Strong organizational and problem-solving skills
โ Ability to manage multiple staffing priorities in a fast-paced environment
โ Excellent communication and relationship-building skills
โญ WITH:
โ
Active STNA, CPR certification, and current TB test
๐ Valid driverโs license with reliable, insured transportation
โญ PREFERRED Experience:
โ Social Worker or Nursing degree is a plus
โ Home care scheduling or caregiver staffing experience
โ Experience with AxisCare or similar home care software
โ Background in home care, hospice, assisted living, or healthcare operations
โ Experience as a Social Worker, Case Manager, Care Coordinator, Discharge Planner, or Patient Advocate

Caregiver Scheduling & Staffing Coordination:
โ Manage caregiver schedules using AxisCare software
โ Ensure full client coverage and prevent missed shifts
โ Respond to call-offs and urgent staffing needs
โ Maintain a dependable backup caregiver pool
โ Monitor overtime and scheduling efficiency
๐ค Client Care Coordination:
โ Match caregivers with clients based on skills, personality, and care needs
โ Understand client care plans and home environments
โ Promote client satisfaction and caregiver retention
โ Act as a liaison between clients, families, caregivers, and healthcare providers
๐งโโ๏ธ Caregiver Support & Field Operations:
โ Address caregiver and client concerns
โ Resolve scheduling and care-related issues
โ Provide field support to caregivers
โ Maintain strong client relationships and continuity of care
๐ Staffing & Caregiver Development:
โ Identify caregiver training needs
โ Coordinate training with the Hospice Support Nurse
โ Recognize staffing gaps and assist with hiring needs
๐ฟ Hospice Coordination:
โ Assign appropriately trained caregivers to hospice clients
โ Coordinate with hospice partners as needed
๐ Field Engagement (Client Care):
โ Provide approximately 12 hours per week of direct client care
โ Support clients and caregivers in the field
โ Assist with coverage when needed
๐ Compliance & Documentation:
โ Ensure caregiver documentation meets required standards
โ Confirm documentation is uploaded in AxisCare
โ Maintain complete caregiver files
โ Support compliance with Ohio non-medical home care regulations
