About the Company
Digital Health Information is a new and innovative company that provides dedicated care to the geriatric community, specifically to patients residing in their own home. As a new ecosystem we are working to expand our operations nationwide, one state at a time, and currently operate in Pennsylvania and New Jersey localities.
OVERVIEW
The Licensed Practical Nurse (LPN) will be part of our clinical care navigator team. They will be responsible for day-to-day front and back-office activities. They assist the physician in conducting telehealth appointments and documenting outcomes to ensure patient success.
